How to Apply

  1. Download and Complete Application
    Download the PT/PTA application packet from the board website. The application must be signed and notarized.
  2. Submit Application Fee
    Pay the $240 application fee via money order or cashier's check made payable to the State of Wyoming. Personal checks and cash are not accepted.
  3. Request Official Transcripts
    Have your official transcripts sent directly from your CAPTE-accredited physical therapy program to the Board office.
  4. Register for the NPTE
    Register for the National Physical Therapy Examination (NPTE) through the FSBPT website and request that scores be sent to Wyoming.

  5. Complete Jurisprudence Exam
    Complete the open-book Wyoming Jurisprudence Examination online. Results are automatically forwarded to the board.
  6. Submit Fingerprint Cards
    Submit two completed FD-258 fingerprint cards for a state and national criminal history background check.
  7. Provide Letters of Recommendation
    Arrange for two professional letters of recommendation to be sent directly to the Board office attesting to your moral character.
