How to Apply

Download and Complete Application ▾Download the PT/PTA application packet from the board website. The application must be signed and notarized.

Submit Application Fee ▾Pay the $240 application fee via money order or cashier's check made payable to the State of Wyoming. Personal checks and cash are not accepted.

Request Official Transcripts ▾Have your official transcripts sent directly from your CAPTE-accredited physical therapy program to the Board office.

Register for the NPTE ▾Register for the National Physical Therapy Examination (NPTE) through the FSBPT website and request that scores be sent to Wyoming.

Complete Jurisprudence Exam ▾Complete the open-book Wyoming Jurisprudence Examination online. Results are automatically forwarded to the board.

Submit Fingerprint Cards ▾Submit two completed FD-258 fingerprint cards for a state and national criminal history background check.

Provide Letters of Recommendation ▾Arrange for two professional letters of recommendation to be sent directly to the Board office attesting to your moral character.
