How to Apply

  1. Complete the Jurisprudence Exam ▾
    Take the online West Virginia Jurisprudence Exam for SLP and Audiology. You must submit the completion certificate with your application.
  2. Initiate FBI Criminal Background Check ▾
    Follow the Board's specific instructions to complete a fingerprint-based FBI criminal background check. Results must be sent directly to the Board.
  3. Download and Complete Application ▾
    Download the 'Initial Application for SLP and AUD' from the Board's forms page. Ensure all sections are completed and the form is signed.
  4. Request Official Documents ▾
    Request official transcripts from your university and Praxis scores from ETS to be sent directly to the West Virginia Board.
  5. Submit Application and Fees ▾
    Mail the completed application, Jurisprudence certificate, and the $200 total fee ($100 application fee + $100 license fee) to the Board office in Buckhannon.
  6. Complete Clinical Fellowship (If Applicable) ▾
    If you are a new graduate, apply for a Provisional License first and complete your 9-month Postgraduate Professional Experience (PPE) under a licensed supervisor.

West Virginia is a member of the ASLP Interstate Compact and is actively issuing compact privileges.

Steps

  1. Verify Out-of-State License ▾
    Provide proof of a current, unencumbered SLP license in another state with equivalent standards.
  2. Submit Initial Application ▾
    Use the standard Initial Application form and check the box indicating you hold an active license in another state.
  3. Complete Jurisprudence and Background Check ▾
    All reciprocity applicants must still complete the WV Jurisprudence exam and the FBI background check.
  4. Submit Fees ▾
    Pay the required application and license fees as specified for reciprocity applicants.
