How to Apply
Complete the Jurisprudence Exam

Take the online West Virginia Jurisprudence Exam for SLP and Audiology. You must submit the completion certificate with your application.
Initiate FBI Criminal Background Check
▾
Follow the Board's specific instructions to complete a fingerprint-based FBI criminal background check. Results must be sent directly to the Board.

Request official transcripts from your university and Praxis scores from ETS to be sent directly to the West Virginia Board.
Submit Application and Fees

Mail the completed application, Jurisprudence certificate, and the $200 total fee ($100 application fee + $100 license fee) to the Board office in Buckhannon.
Complete Clinical Fellowship (If Applicable)

If you are a new graduate, apply for a Provisional License first and complete your 9-month Postgraduate Professional Experience (PPE) under a licensed supervisor.

Provide proof of a current, unencumbered SLP license in another state with equivalent standards.

All reciprocity applicants must still complete the WV Jurisprudence exam and the FBI background check.
