- Affidavit of Training Completion — A signed affidavit from the Pharmacist-in-Charge (PIC) verifying completion of the 500-hour training program.
How to Apply

Register as a Pharmacy Technician Trainee ▾If you are not yet certified, you must first register as a trainee to begin your on-the-job training. This allows you to work under the direct supervision of a pharmacist.

Complete a Board-Approved Training Program ▾Complete either a 500-hour employer-based on-the-job training program or a Board-approved academic pharmacy technician program.

Pass a National Certification Exam ▾Successfully pass the Pharmacy Technician Certification Exam (PTCE) from PTCB or the ExCPT from NHA to earn your CPhT credential.

Submit Fingerprints for Background Check ▾Schedule a fingerprinting appointment through IdentoGO using service code 228QQN. Results must be sent directly to the Board.

Apply for Full Technician Registration ▾Once certified and training is complete, submit the full Pharmacy Technician application along with the $25 fee and a 3x4 head-and-shoulders photograph.
