- NPDB Self-Query Report — A current digital self-query report from the National Practitioner Data Bank, emailed to the Board. NPDB Website
- Official Transcripts — Final transcripts showing the conferral of a doctoral degree (AuD) sent directly from the educational institution.
- Praxis Exam Scores — Official scores from the national examination in audiology sent via ETS using code 9347.
- ASHA CCC or ABA Certification — Verification of current Certificate of Clinical Competence (CCC-A) or ABA certification. The Board can often download this if the number is provided.
- License Verifications — Official verification of all health-related licenses ever held in any other U.S. jurisdiction.

Submit Application and Fee ▾Complete the online application for Audiologist licensure and pay the non-refundable application fee of $135 ($50 for provisional).

Request NPDB Self-Query ▾Request a digital self-query report from the National Practitioner Data Bank (NPDB). Once received, email the digital report directly to audbd@dhp.virginia.gov.

Submit Praxis Scores ▾Request that your Praxis exam scores be sent electronically to the Virginia Board using Board Code 9347.

Requirements ▾

Verify Active Practice ▾Provide evidence of active practice for at least one of the past three years preceding the application date.
