Slow Downs

Incomplete Practical Training Affidavit
Missing signatures or failing to meet the 180-hour minimum requirement will result in application denial.
Fingerprint Rejection
Poor quality prints can be rejected by the FBI/BCI. Ensure prints are taken by an approved vendor to minimize this risk.
Missing BCI Report for Trainees
Trainee applicants often forget to include the original hard-copy BCI report, which is required for those 18+.
Unapproved Training Programs
Using a training program not recognized by the Utah Board of Pharmacy will lead to immediate application rejection.
