- OHA Pain Management Certificate — Completion certificate from the mandatory 1-hour Oregon Pain Management Commission module.

How to Apply

Create Applicant Portal Account ▾Register for an account on the Oregon Occupational Therapy Licensing Board (OTLB) Applicant Portal to start your application.

Submit Application and Pay Fees ▾Complete the online application for Occupational Therapist Registered (OTR). The initial fee for OTs is $330, which includes the $50 fingerprint background check fee.

Complete Fingerprinting ▾Schedule a fingerprinting appointment through Fieldprint Oregon using the agency code: FPOccuTheraLicBd. Results are sent directly to the Board.

Pass the Oregon Law/Ethics Exam ▾Take the mandatory open-book online Law/Ethics exam. You must pass this exam before a license can be issued.

Complete OHA Pain Management Module ▾Complete the free 1-hour Oregon Health Authority (OHA) Pain Management Module and upload the certificate to your application portal.

Licensees must complete 30 points of CE every two years. Maintaining active NBCOT certification satisfies the 30 points, but licensees must still complete mandatory cultural competency and pain management courses.
