What You'll Need

• Official Transcripts — Must be sent directly from the educational institution to the Board.
• NBCOT Score Report — Verification of passing the OTR exam sent directly from NBCOT.
• Evidence of Status Form — Required to verify citizenship or qualified alien status.
• Oath and Photo Page — A notarized document including a recent photograph of the applicant.
• Chronological History — A detailed history of all education and employment from the date of graduation to the present.

How to Apply

1. Graduate from an Accredited Program ▾
   Complete an occupational therapy educational program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE).

2. Pass the NBCOT Exam ▾
   Register for and pass the National Board for Certification in Occupational Therapy (NBCOT) OTR examination.
3. Submit Online Application ▾
   Create an account on the Oklahoma Medical Board portal, select 'Occupational Therapist', and pay the $120 application fee.
4. Request Official Transcripts ▾
   Have your university send official transcripts directly to the Oklahoma Board of Medical Licensure and Supervision.
5. Complete Extended Background Check (EBC) ▾
   Complete the online EBC Authorization Form to initiate the required criminal history background check.
6. Submit Required Forms ▾
   Download and submit the Evidence of Status form, Oath and Photo page, and any necessary supervision forms (Form 5).

7. Board Review ▾
   Applications must be complete 30 days prior to a scheduled Occupational Therapy Advisory Committee meeting for review.
