Endorsement Steps

Step 1
Complete Education & Supervised Practice
Earn a master's degree or higher from an ACEND-accredited program. Complete between 1,000 and 1,200 hours of board-approved supervised practice or an accredited dietetic internship.

Pass the CDR Examination
Successfully pass the Commission on Dietetic Registration (CDR) national examination.

Submit the Online Application
Apply through the Oklahoma State Board of Medical Licensure online portal and pay the $120 application fee.

Complete Background Check & Provide Documents
All applicants must submit to an Extended Background Check. You must also submit primary source verification of your education and passing CDR exam score.

License Types
Licensed Dietitian (LD): The credential required to practice dietetics in the state of Oklahoma. Supervised by the Licensed Dietitians Committee under the Medical Board. Note: Oklahoma does not license the title 'Nutritionist'.
Provisional Licensed Dietitian (PLD): Available for applicants who have completed the academic and supervised practice requirements but have not yet passed the CDR examination. Must practice under the supervision of a Licensed Dietitian. Valid for one year.

- CDR Registration Verification: Proof of active registration with the Commission on Dietetic Registration sent directly to the Board.
- Official College Transcripts: Sent directly from the university showing your conferred degree.
- Extended Background Check: Processed through the Oklahoma State Bureau of Investigation (OSBI).
- Evidence of Legal Status: Form documenting your citizenship or qualified alien status.
