How to Apply

Obtain National Registration ▾You must be a Registered Dietitian (RD) with the Commission on Dietetic Registration (CDR) before applying for a full license in Oklahoma.

Complete Online Application ▾Submit the initial application for licensure through the Oklahoma Medical Board's online portal. Select 'Licensed Dietitian' (LD) or 'Provisional Licensed Dietitian' (PD).

Pay Application Fees ▾Pay the non-refundable application fee ($120 for LD, $30 for PD) online via credit card or electronic check.

Submit Required Forms ▾Download and mail the Oath and Photo page, Evidence of Status form, and the Extended Background Check (EBC) Consent form to the Board office.

Requirements ▾
Applicants currently licensed in another state may apply via endorsement if the other state's licensing requirements are substantially equivalent to or stricter than Oklahoma's. The standard application, fee, and verification of all out-of-state licenses are required.
