How to Apply

  1. Create an eLicense Account
    Register for a new account on the Ohio eLicense portal to begin the application process.
  2. Submit Initial Application
    Log in to the eLicense portal, select the 'Audiologist' license type, and complete the online application form.

    Pay the non-refundable $200 application fee plus a $3.50 portal maintenance fee via credit card (Visa, MasterCard, or Discover).
  4. Complete Background Checks
    Complete both BCI and FBI criminal records checks. Use reason code R.C. 4753.061 and request results be sent directly to the Board.
