What You'll Need

• Official Transcripts — Must be sent directly from the accredited DPT program to the Board.
• NJ JAM Score Report — Proof of passing the New Jersey Jurisprudence Assessment Module, typically sent automatically by FSBPT. NJ JAM Info
• NPTE Score Report — Official scores sent directly from FSBPT to the New Jersey Board.
• Passport-Style Photograph — A recent 2x2 color photograph of the applicant's head and shoulders, uploaded during the online application.

How to Apply

1. Submit Online Application ▾
   Create an account on the New Jersey MyLicense portal and complete the Physical Therapist application. Pay the non-refundable $125 application fee.
2. Request Official Transcripts ▾
   Arrange for your DPT program to send official transcripts directly to the Board. They can be emailed to physicaltherapy@dca.njoag.gov or mailed to the Board office.

3. Complete the NJ JAM ▾
   Register for and pass the New Jersey Jurisprudence Assessment Module (NJ JAM) through FSBPT. A passing score of 80% is required.

4. Register for the NPTE ▾
   Register with FSBPT for the National Physical Therapy Examination. The Board must approve your eligibility before you receive your Authorization to Test (ATT).

5. Complete Fingerprinting ▾
   After the Board reviews your application (typically 3-7 business days), you will receive a letter with a service code to schedule your fingerprinting appointment with IdentoGO.

6. Pay Initial License Fee ▾
   Once all requirements are met and you are approved, the Board will notify you to pay the initial license fee ($110 if in the first year of the cycle, $55 if in the second).
