What You'll Need

  • Completed Application Form — Notarized application for licensure as an Audiologist.
  • Passport Photos — Two (2) 2x2 passport-size photographs taken within the last six months.
  • Official Transcripts — Transcripts showing completion of a doctoral degree in Audiology from an accredited institution.
  • Praxis Exam Scores — Official score report from ETS for the Audiology Praxis exam.
  • Clinical Internship Proof — Statement from supervisor or a copy of ASHA CCC-A certification if internship was completed more than a year ago.

How to Apply

  1. Submit Initial Application
    Complete the paper application form and mail it to the Committee with the $75 non-refundable application fee (check or money order).
  2. Submit Educational Transcripts
    Arrange for official transcripts from your doctoral program (AuD) to be sent directly from the university to the Committee.
  3. Provide Praxis Scores
    Request that ETS send your official Praxis Audiology exam scores directly to the New Jersey Board.

  4. Complete Criminal History Background Check
    After the Board receives your application, they will mail you specific fingerprinting instructions and a contributor code for IdentoGO.

  5. Complete Jurisprudence Orientation
    Complete the mandatory online Jurisprudence Orientation covering New Jersey statutes and regulations.

    Once approved, you must pay the initial license fee ($170 if in the first year of the biennial cycle, $85 if in the second) to receive your license number.

No. As of 2022, licensed audiologists in New Jersey may dispense hearing aids under their audiology license.

All audiology licenses in New Jersey expire on October 31 of odd-numbered years.
