How to Apply

  1. Download and Complete Application
    Download the official Pharmacist Application PDF. Select your method of licensure: Examination, Score Transfer, or Reciprocation.
  2. Submit Fingerprints
    Complete a fingerprint card and the Nevada Department of Public Safety's Background Waiver Form. Mail these with a separate $40.25 fee to the Board.

    Submit the non-refundable $250 application fee. Payments can be made by check, money order, or credit card (5% processing fee applies for cards).
  4. Register for NAPLEX
    If applying by examination, register for the NAPLEX through the NABP website. Ensure your transcripts are sent directly to NABP.

    Starting May 2025, Nevada uses the NVLE instead of the MPJE. After your application is processed, the Board will email instructions to access this online, non-proctored exam.
