Complete Background Study

New applicants must initiate an online background study through the Minnesota Department of Human Services (DHS) NETStudy 2.0 system. Use provider number HP2Ap.
Create MDH Licensing Account
▾
Register for an account on the Minnesota Department of Health (MDH) Licensing System portal to begin the online application process.
Submit Initial Application
▾
Complete the online application for Audiologist licensure. You may apply for a temporary license if you need to practice while your full application is processed.
Submit Required Documentation
▾
Provide official transcripts, verification of clinical training, and ASHA/ABA certification if applicable. Documents can be uploaded or mailed to MDH.
