What You'll Need

• Certification of Pharmacy Education — Form LARA/CERTPHARMED must be completed by your school and sent directly to the Board.
• Official Transcripts — Official transcripts showing the degree conferred must be sent directly from the educational institution.
• NABP Exam Scores — Passing scores for NAPLEX and MPJE must be released to Michigan via NABP. NABP Exams
• Implicit Bias Training Proof — Documentation of at least 2 hours of implicit bias training completed within the 5 years prior to application.
• Human Trafficking Training Proof — Attestation or proof of completion of a one-time training in identifying victims of human trafficking.

How to Apply

1. Register with NABP ▾
   Create an e-Profile with the National Association of Boards of Pharmacy (NABP) and register for the NAPLEX and MPJE exams.

2. Apply for Michigan Eligibility ▾
   Apply for exam eligibility through your NABP e-Profile. Michigan is a board that confirms eligibility through NABP.

3. Submit MiPLUS Application ▾
   Create an account in the Michigan Professional Licensing User System (MiPLUS) and submit the Pharmacist by Examination application along with the required fees.
   Apply via MiPLUS
4. Complete Fingerprinting ▾
   Wait for the Application Confirmation letter from LARA containing your Customer ID. Then, schedule and complete fingerprinting through IdentoGO.
5. Submit Educational Verification ▾
   Arrange for your pharmacy school to send the 'Certification of Pharmacy Education' form and official transcripts directly to the Michigan Board of Pharmacy.
6. Complete Mandatory Training ▾
   Ensure you have completed the one-time Human Trafficking training and the required Implicit Bias training hours before license issuance.
