What You'll Need

How to Apply

  1. Submit Initial Application
    Download and complete the 'Pharmacist Initial Application' for U.S. or Foreign Graduates. Mail the paper application with the $150 fee to the Board's Lock Box address.

  2. Complete Criminal Background Check
    Obtain a state and federal criminal history records check through the Maryland Criminal Justice Information System (CJIS). Use Authorization Number 0600062013.
  3. Register for Exams with NABP
    Create an e-Profile with the National Association of Boards of Pharmacy (NABP) and register for the NAPLEX and MPJE exams.

  4. Receive Authorization to Test (ATT)
    Once the Board verifies your eligibility, NABP will issue an ATT via email. Use this to schedule your exam dates through Pearson VUE.
  5. Pass Required Examinations
    Successfully pass both the NAPLEX and the Maryland MPJE with a score of 75 or higher. Scores are valid for one year.
  6. Final Licensure
    After passing exams and meeting all requirements, the Board will issue your license. You can verify your status online before the physical card arrives.
