Outdated Application Forms
▾
The Board strictly rejects outdated versions of application forms. Always download the most recent PDF from the official website before mailing.
Incomplete Background Checks
▾
Maryland only accepts background checks from the CJIS Central Repository. Using third-party vendors not authorized by CJIS will cause significant delays.
Missing School Seals
▾
Affidavits from pharmacy schools must include the official school seal; photocopies without a visible seal are often rejected.
Unpaid NABP Fees
▾
The Board cannot release your Authorization to Test (ATT) until you have paid the separate exam fees directly to NABP.
