What You'll Need
- Official Transcripts — Must be sent directly from the accredited PT program to the board via mail or secure email (Transcripts@laptboard.org).
- Passport-Sized Photograph — One 2x2 inch color photo taken within the last 6 months, uploaded to the dashboard or emailed to Licensing@Laptboard.org.
- Identification Documents — Copies of two forms of ID: a Social Security card (required) and a Driver's License or Passport.

How to Apply

Create a User Account ▾Visit the board homepage and select 'Sign up!' to create a new applicant account. This dashboard will be used to manage your application and track progress.
Sign Up / Login -
Submit Online Application ▾Complete the online application form within your dashboard. You must provide your FSBPT ID and pay the non-refundable application fee ($250-$390 depending on birth year).
Application Instructions -
Pass the Jurisprudence Exam ▾Take the online Jurisprudence Exam through your dashboard. It consists of 40 questions based on the Louisiana Practice Act and Rules; a score of 75% or higher is required.

Complete Background Check ▾Schedule a fingerprint appointment via IdentoGO using service code 27N3X2. This must be done after the online application is submitted.

How many continuing education hours are required for renewal?
