What You'll Need

• Application for Credentialing — The official state application form, signed and notarized where required.
• Passport-Style Photo — A clear, color head-and-shoulders photograph taken within the last 6 months.
• Official Transcripts — Must be sent directly from the registrar of your CAPTE-accredited program to the Board.

How to Apply

1. Complete Education ▾
   Graduate from a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE).
2. Register for the NPTE ▾
   Register for the National Physical Therapy Examination (NPTE) through the Federation of State Boards of Physical Therapy (FSBPT) and pay the required exam fee.

3. Submit Background Check ▾
   Schedule a fingerprinting appointment through IdentoGO using the Kentucky Board of Physical Therapy service code 27GK3Q. This process can take up to 8 weeks.

4. Submit State Application ▾
   Complete the Kentucky Application for Credentialing and submit it with the $225 application fee and a passport-style photo.
5. Take Jurisprudence Exam ▾
   Complete the free, online, open-book Kentucky Jurisprudence Exam covering state laws and regulations. This is required for initial licensure.
6. Request Transcripts and Scores ▾
   Have your official transcripts sent directly from your educational institution to the Board and request your NPTE scores be transferred via FSBPT.
7. Schedule Exam ▾
   Once the Board approves your application, you will receive an Authorization to Test (ATT) from FSBPT. Use this to schedule your exam date with Prometric.
