How to Apply
Obtain Required Education

Complete a Doctorate degree in Audiology (AuD) from a program accredited by the Council for Academic Accreditation (CAA) or the Accreditation Commission for Audiology Education (ACAE).
Pass the Praxis Exam
▾
Register for and pass the national Praxis examination in Audiology. Scores must be sent directly to the Kentucky Board from ETS.

Complete the Application Form

Download and complete the 'Application for Licensure as a Speech-Language Pathologist or Audiologist'. Ensure all sections, including the affidavit, are signed and notarized where required.
Submit Fees

Include a check or money order for $150 ($50 application fee + $100 licensure fee) payable to the Kentucky State Treasurer.
Request Official Transcripts
▾
Contact your university registrar to have official transcripts conferring your degree sent directly to the Board office.
Submit ASHA CCC Verification (Optional)
▾
If you hold ASHA CCC-A, request that ASHA send verification directly to the Board to satisfy certain clinical experience and exam requirements.
