How to Apply
Create a Portal Account

Register for an account on the Iowa Department of Inspections, Appeals, and Licensing (DIAL) Professional Licensure Portal.
Submit Online Application

Log in and select 'Apply for a New License'. Choose 'Audiologist' and complete the personal and professional information sections.
Pay Application Fee

Pay the non-refundable $120 application fee through the online portal using a credit or debit card.
Complete Background Check
▾
Follow the instructions provided in the portal to complete a fingerprint-based criminal background check. This is required for all initial professional licenses.
Submit Primary Source Verifications
▾
Request official transcripts, Praxis scores, and clinical experience verifications to be sent directly to the Board. If you hold ASHA CCC-A, you may submit that verification instead to satisfy these requirements.
