How to Apply

  3. Complete Criminal Background Check ▾
    After submitting your application, schedule a fingerprinting appointment with IdentoGO using the service code provided by the PLA. Do not do this before applying.
  4. Request Official Transcripts ▾
    Contact your educational institution to have official transcripts sent directly to the Indiana Professional Licensing Agency (PLA).
  5. Register for the NPTE ▾
    Register for the National Physical Therapy Examination (NPTE) through the Federation of State Boards of Physical Therapy (FSBPT) and pay the required exam fees.
