What You'll Need

How to Apply

  1. Create an Access Indiana Account
    Register for a single sign-on account through Access Indiana to access the MyLicense One portal.

  2. Submit Online Application
    Complete the initial Audiologist application via the MyLicense portal and pay the $150 non-refundable application fee.

  3. Complete Criminal Background Check
    After submitting your application, you will receive an email with a service code. Use this code to schedule a fingerprinting appointment with IdentoGO. Do not complete this before applying.

  4. Submit Educational and Clinical Documentation
    Request official transcripts showing a doctoral degree and certification of 1,820 clock hours of supervised clinical practicum. If you hold ASHA CCC-A, you may submit a verification letter in lieu of these documents.

  5. Pass the Jurisprudence Examination
    Once your application is reviewed and approved, the Board will email you the jurisprudence exam and instructions. You must pass with a score of at least 75%.
  6. Verify National Exam Scores
    Request that your Praxis exam scores be sent directly from ETS to the Indiana Professional Licensing Agency.
