Download and Complete Application ▾Download the 'Application for Registration – Pharmacy Technician' (Form PHT-00). Complete all sections legibly in dark ink and sign the declaration.

Determine Correct Fee ▾The fee depends on the year the registration takes effect. Pay $130 if the registration is effective in an even-numbered year, or $80 if effective in an odd-numbered year. Make checks payable to 'Commerce and Consumer Affairs'.

Submit Application by Mail ▾Mail the completed application and check to: Board of Pharmacy, DCCA, PVL Licensing Branch, P.O. Box 3469, Honolulu, HI 96801. Online applications are not accepted for this registration type.

Optional: Apply for Vaccine Administration ▾If you intend to administer vaccines, you must also submit the 'Vaccine Administration Application'. This requires national certification (PTCB or NHA) and a $10 fee.
