How to Apply

  1. Create MQA Online Account
    Register for an account on the Florida Health Source (MQA) portal to begin the electronic application process.
  2. Submit Application and Fees
    Complete the online application for Pharmacist Licensure by Examination and pay the $295 total fee ($100 application, $190 initial license, $5 unlicensed activity).
  3. Complete Electronic Fingerprinting
    Schedule a Livescan fingerprinting appointment using ORI number EDOH4680Z. Results are sent directly to the Board.

  4. Register for NAPLEX and MPJE
    Apply for exam eligibility through the National Association of Boards of Pharmacy (NABP). Florida requires both the NAPLEX and the Florida MPJE.

  5. Submit Educational Credentials
    Request your pharmacy school to send an official transcript or 'Form A' (Certificate of Pharmacy Education) directly to the Florida Board of Pharmacy.
  6. Verify Internship Hours
    If you graduated after Jan 1, 2001, with a PharmD, your degree satisfies the 2,080-hour requirement. Otherwise, submit Form B to certify internship hours.
