What You'll Need
- Form A (Certificate of Pharmacy Education) — Must be completed by the Dean or Registrar of your pharmacy school and sent directly to the Board.
- Official Transcripts — Required if Form A is not used; must show degree conferral and be sent directly from the institution.
- Form B (Internship/Work Experience) — Required for those who graduated before 2001 or are using work experience to satisfy the 2,080-hour requirement.
- License Verification — If you hold or have ever held a license in another state, you must request an official verification be sent to Florida.

Create MQA Online Account ▾Register for an account on the Florida Health Source (MQA) portal to begin the electronic application process.

Submit Application and Fees ▾Complete the online application for Pharmacist Licensure by Examination and pay the $295 total fee ($100 application, $190 initial license, $5 unlicensed activity).

Complete Electronic Fingerprinting ▾Schedule a Livescan fingerprinting appointment using ORI number EDOH4680Z. Results are sent directly to the Board.

Register for NAPLEX and MPJE ▾Apply for exam eligibility through the National Association of Boards of Pharmacy (NABP). Florida requires both the NAPLEX and the Florida MPJE.

Submit Educational Credentials ▾Request your pharmacy school to send an official transcript or 'Form A' (Certificate of Pharmacy Education) directly to the Florida Board of Pharmacy.
