How to Apply

    Register for an account on the Florida Department of Health Medical Quality Assurance (MQA) Services Portal to begin the application process.
  2. Submit Application and Fees
    Complete the online application or mail form DH-SPA-1. Pay the non-refundable application fee and initial licensure fee ($280 total for full cycle, $180 for reduced cycle).

  3. Complete Electronic Fingerprinting
    Register with a Livescan service provider using ORI number EDOH4740Z. You must generate a Livescan Request Form via the CHAI system.

    Request your university to send official transcripts directly to the Board office showing conferral of a doctoral degree in audiology.
  5. Submit Praxis Exam Scores
