How to Apply

  1. Verify Eligibility ▾
    Ensure you are a graduate of an ACPE-accredited pharmacy program (or have FPGEC certification) and meet legal residency requirements.
  2. Obtain Arizona Fingerprint Clearance Card ▾
    Apply for a Fingerprint Clearance Card through the Arizona Department of Public Safety (DPS). Select 'Regular Application - Paid Employee' and 'Board of Pharmacy - Licensure'.

  6. Pass Examinations ▾

Transfer License

Requirements ▾
