How to Apply

  2. Submit Initial Application
    Complete the Audiologist License application (Form 08-4056) either online via the portal or by mailing the paper form.

  3. Pay Application and License Fees
    Pay the non-refundable $200 application fee and the $70 initial license fee. A separate fingerprint processing fee is also required.

  4. Complete Fingerprinting
    As of August 31, 2024, all applicants must submit one original FD-258 fingerprint card for a criminal background check.

  5. Submit Official Transcripts
    Request official transcripts showing a master's or doctoral degree in audiology to be sent directly from your educational institution to the Board.
  6. Verify Clinical Competency
